The National Wildfire Coordinating Group is the federal supplier for ICS training and support materials. They do not accept phone orders. They have started a web presence and are working at online ordering. They do accept Visa and Mastercard.
There is a charge for the catalogs and you do need to fax or mail in the order for them. Here's the contact information that you need to get started:

Volcanic Ash Avoidance Training Materials
The ICAO distributes a poster and video for aviators on how to deal with volcanic ash. The video was produced by Boeing and the ICAO is the exclusive distributor. Don't call Boeing to ask for it since they don't have a clue. The price is subject to change but the video is about $50.00 US.
